2 / Import procedure
a) For medical equipment classified as A: Only imported after receiving the receipt of application for announcement of applicable standards issued by the Department of Health of provinces and cities, in addition to the customs dossier Please provide the following:
– A list of medical equipment according to the form in Appendix V issued with Decree No. 36/2016 / ND-CP or papers proving the results of classification of medical equipment according to the provisions in the Circular Circular No. 42/2016 / TT-BYT dated November 15, 2016 providing for recognition of results of classification of medical equipment (Submit consularly legalized copy or certified copy of legalized copy consular chemistry).
– Power of attorney of the circulation number owner (in case the importing unit is not the owner of the circulation number).
b) For medical equipment classified as B, C, D and on the list of medical equipment issued together with Circular No. 30/2015 / TT-BYT, when being imported, the following must be attached: :
– Having an import license issued by the Ministry of Health.
– Classification of medical equipment
c) For medical equipment classified as B, C, D and not on the list of medical equipment issued together with Circular No. 30/2015 / TT-BYT: When importing, only: the prescribed classification.
